Meaning
Shorthand for "I am listening on a higher frequency than I am transmitting on" — used when working split. "DOWN" is the equivalent for listening below the transmit frequency.

Practical usage
"UP 5" means listening 5 kHz higher than the transmit frequency; a bare "UP" without a number is genuinely ambiguous and considered less helpful practice than specifying an exact offset or range (e.g. "UP 5 TO 10").
